| Description (Service) | Annual appointments to review complex prescriptions with a pharmacist * pharmacists discuss medications, side effects, interactions and how to administer them MedsCheck At Home - Available to people who cannot get to the pharmacy due to illness, frailty, old age, or because of isolation * pharmacist visits eligible patients at home to discuss medications * pharmacist also aids with disposal of unused and expired medication |
