| Description (Service) | This Halton Healthcare led initiative, through CANES Community Care and supported by Ontario Health AtHome, offers a solution to enable patients to return home from hospital to await their long-term destination
Based on assessed care needs, patients will have access to enhanced PSW supports in the home, for up to 60 days
