Referral program that links individuals and families affected by Alzheimer's disease or related dementia to a community of learning, services and support
- provides access for individuals living with dementia and their caregivers to comprehensive and coordinated services
- people and families are connected with support from a counsellor when a First Link Referral is made by health care professionals or community agencies
- caregiver support groups and counselling sessions
- increased effective and efficient utilization of community resources
- one-on-one non-therapeutic counselling and group supports for caregivers, including family and friends
