Description and Service Details
| Description (Service) | Six-week program * exercise and falls prevention program for older adults with multiple medical conditions * aims to restore their functional abilities, improve their quality of life, and allow them to live independently or with the support of a caregiver in a community setting
Client can enroll in the program if they experience one or more of the following:
