Description and Service Details

Description (Service) Exercise program to older adults in their own home * 12-week program * sessions are provided once a week by trained volunteers through a series of 12 simple and progressive exercises * designed to improve strength, balance, and mobility for less mobile older adults while preventing falls

Eligibility criteria:
  • able to follow instructions or have a caregiver willing to assist and complete exercises independently (not appropriate for mid to late stage dementia/cognitive impairment))
  • frailty
  • declining mobility
  • lives in own home or a retirement home
  • unable to leave home to attend community physical activity programs
  • not currently receiving physiotherapy
  • willing to try HSEP
