| Description (Service) | Develops and delivers client-focused community health, support and volunteer services * assists people with health care needs in their own home and community Nursing services include: Visiting nursing
Depending on the type of nursing care, patients can be treated at one of three nursing clinics in Burlington, Oakville or Milton * the clinics are for Acclaim Health's Ontario Health atHome Mississauga Halton patients only and are not open to the general public
Personal Support Workers (PSWs) provide home and personal care to those who are elderly * recovering from surgery * illness or injury * living with dementia and other cognitive impairments * adults and children with special needs * facing terminal illness * minimum booking is three hours of care Services include:
