Description and Service Details
Description (Service) | Provides a range of non-medical, assisted living services to support high risk seniors with complex needs to continue living independently in the comfort of their own homes, including personal hygiene activities (washing, bathing, hair care, mouth care, preventative skin care, transferring/ positioning, etc.), personal routine of activities of daily living (dressing/undressing, toileting, help with eating, medication reminders), and essential homemaking services and safety and reassurance checks * services are scheduled at multiple times throughout the day or overnight according to clients' needs and preferences * each scheduled time is customized to the client's needs, depending on the level of support needed and the activity involved Service providers are:
SDL services are designed for seniors who need shorter, more frequent visits to help prevent premature placement in long-term care * clients may be eligible for up to 1.5 hours of personal support each day Types of services provided:

Eligibility | 65 years of age or older * insured under OHIP * demonstrates a need for intermittent Personal Support/Attendant Services provided throughout a 24hr period * InterRAI-HC or InterRAI-CHA Assessment MAPLe score of 3 or higher * personally able to direct their own care and communicate their needs (with or without aides) * medically stable and pose no risks to themselves or others * able to be left alone between visits * agreeable/compliant with their service plan * personal care/support required equals approximately 1.5hrs per day |
