Description and Service Details
| Description (Service) | Provides assessment, education, treatment and support for people with diabetes, pre-diabetes, and people that are at risk of developing Type 2 diabetes. Also offers one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices.
