Description (Service)

Provides a Full-Continuum Model of Care, planned and designed according to needs of the patients

  • provides early detection and early intervention
  • removes barriers and the unnecessary challenges for patients and families trying to navigate a complex system 

Adult Mental Health Programs & Services:

  • provides a continuum of care from assessment or consultation, to specialized treatment supports for clients with complex and serious mental health disorders

Mental Health Inpatient Unit:

  • a 19 bed unit serving patient 18 years of age and older with a focus on short-term crisis intervention, psychiatric assessment/treatment and stabilization

Innovative Programs and Services:

  • Prioritizing Health through Acute Stabilization and Transition (PHAST) is a made-in-Burlington service delivery innovation at the heart of a broader Full-Continuum Model for Mental Health and Addictions services
  • Program of Assertive Community Treatment (PACT) is a community outreach program, serving residents of the Region of Halton

Psychiatric Emergency Services:

  • offers emergency assessments, care coordination and a follow up plan of safe care for any individual in a psychiatric crisis situation

Urgent Care & Outpatient:

  • a community-based crisis alternative for rapid consultation/assessment, medication/symptom review and stabilization follow up for up to six weeks for adults 18-65 years whose psychological or psychiatric distress is escalating and requires prompt intervention

Youth & Adolescent Mental Health Programs & Services

  • Child and Adolescent Psychiatric (CAP) provides early intervention support to children/youth under the age of 18 years
  • Youth Early Intervention (YEI) program is youth-centred and breaks down barriers that youth encounter when seeking mental health and/or substance use services
