Description (Service)

The Halton office is the contact point for families/individuals who are interested in accessing:

  • Supported living services and supports
  • Caregiver respite
  • Community participation supports (with social and recreational activities, work activities and volunteer activities, including Passport)
  • Specialized services, such as service coordination or clinical services
  • Person-directed planning supports

Mon-Fri 9 am-4:30 pm

Eligibility Ages: 18 year(s) and up 
 
individual must have a documented developmental disability (documentation accepted is a psychological assessment) and meet age and residency requirements
Application

Documents needed to complete an application:

Document confirming age:

  • copy of a birth or baptismal certificate
  • passport
  • drivers licence

Proof of Canadian citizenship, landed immigrant status or permission to stay by Citizenship and Immigration Canada:

  • copy of passport
  • naturalization certificate
  • immigration documents
  • Minister's permit

Documents confirming Ontario residency include but are not limited to:

  • rental or lease agreement
  • statement of direct deposit for Ontario Disability Support Program
  • employer record (pay stub or letter from employer on company letterhead)
  • mailed bank account statements (not automated teller receipts or bank books)
  • utility bill
