Description (Service)

The program aims to improve the health, wellness and quality of life for seniors by improving access to assessment and treatment of incontinence (including urinary incontinence, fecal incontinence, urinary frequency day or night, problems with bowel control, frequent urinary tract infections, difficulty learning how to use or care for a urinary catheter, and constipation) * patients will be assessed by a Nurse Continence Advisor who is registered nurse with advanced education and training in continence care

Administration: Mon-Fri 8:30am-4:30pm * appointment required for home visits

Areas Served Etobicoke South ; Halton Hills ; Milton ; Mississauga ; Oakville
Boundaries Mississauga and Toronto (Etobicoke)
Eligibility 60 years of age and older experiencing continence related issues and resides within the Mississauga Halton area

Self-referral * physician or nurse practitioner's referral is required for Regional Continence Home Visits
