Description and Service Details
Description (Service) Palliative care outpatient program comprises of a palliative physician (Mon-Thu), a nurse practitioner (full-time), a social worker (half-time) and a dietitian (part-time) * any one or all team members may be consulted * the team collaborates with all health care providers involved in the client's care  
 
Care may be provided in the home, hospice, long-term care facility or outpatient clinic at Joseph Brant Hospital * people living with cancer may be seen earlier in their disease phase 
 
A palliative care clinic (by appointment only) is held once a week at the cancer clinic in Joseph Brant Hospital to address pain and symptom management issues
Hours Office: Mon-Fri 8:30 am-4:30 pm

Individuals and their family members living with a life-threatening illness
Application Referrals to the clinic can come from a variety of sources including doctors and other health-care professionals
